Self Referrals – For Adults Only

If you are an adult that has watched our webinars and you are registered with a GP in the county of Somerset in the United Kingdom and you feel that you would benefit from further support from a Dietitian then please self-refer using one of the self-referral forms below.

You may either download and print the form to complete, or fill it in electronically, depending on your preference.

Please note that generally our dietitians will only be able to offer you one appointment for an assessment and a nutritional care plan, with the expectation that you will seek follow-up via your GP as required.
