Self Referral Form – Only for Patients with a Somerset GP
Please complete the self referral form and food diary and then post/email as per the details on the reverse of the form.
Please note that a recent (within the past 2 months) weight and height for your child need to be included otherwise the referral will not be accepted.
If your GP is out of the area please contact your local Dietetic Department for advice.
Unless you have been advised differently in your paediatric dietetic correspondence, please complete the downloadable self referral form after 4-6 months of trying the suggestions in the webinars if you feel that you require further advice.
